Root Cause Analysis (RCA) Tool Kit

Purpose

This tool kit contains tools and resources for education and use during Root Cause Analysis (RCA) processes. RCA is a retrospective (looking back) systematic approach to understanding the causes of an adverse event and identifying system flaws that can be corrected to prevent the error from happening again. It is not appropriate in cases of negligence or willful harm.
